Memorandum of Intention

This form is required to begin the process to make a stock donation. This form must be completed before you make any stock transfer. Once completed and submitted to the United Methodist Foundation, a follow-up email with account information will be sent to the email address you provide on this form.* Please provide this information to your broker to complete the transaction. NOTE: All gifts of securities must be DTC/ACAT eligible. If donating a mutual fund, please contact our office at (919) 836-0029. Some mutual fund donations require different instructions and your donation may be significantly delayed if you attempt to send the shares to the DTC number you will receive upon submission of your Memorandum of Intention.

PLEASE NOTE: The Foundation reserves the right to approve the intended recipients, making sure that the donor’s wishes are in alignment with the mission of the Foundation. Securities will be sold on the next available settlement day as soon as the Foundation is made aware of the transfer. The Foundation will inform the donor of the total settlement of securities that are donated; however, donors should consult with their tax advisor or CPA to determine the allowable charitable deduction for tax purposes. The Foundation is not responsible for a sudden increase or decrease in value of securities and/or the timing of when securities are sold.
